Manchester Mind works in partnership with Greater Manchester Mental Health Trust to deliver an Assertive Outreach Service across Manchester. The Manchester Engagement Team is a multi-disciplinary team with staff employed both by Manchester Mind and the Trust in order to provide an innovative service to a difficult to engage client group.
As a Social Worker you will be a crucial member of the team. If successful, you would be expected to proactively support and work with hard to reach service users, to care co-ordinate a caseload of service users with severe and enduring mental health problems and complex needs; to implement and record care plans and risk assessments and review them in collaboration with service users; to support an approach to the delivery of care with an ethos of team work, providing both specialist and generic interventions and act as a skill resource for the rest of the team; to liaise with and provide support to carers; to liaise with other statutory and non-statutory agencies to ensure a comprehensive package of care is provided and to ensure that the service user is included in all decision making about their life.
Manchester Mind is striving to be an equal opportunities employer and welcomes applications from all sections of the community. Manchester Mind sees it as a positive advantage if you have experience of mental health issues and/or have used mental health services.
